There is also a WORD  document attached below which details the step by step giude to processing a claim in the club.

Submit your initial data within 60 days of your injury else you will be ineligble







These 4 forms are also required depending on what you are claiming for i.e. Medical expenses or loss of wagers or both

>> ALL CLAIMS FOR GAA SCHEME  (mens/boys games)  NOW NEED TO BE PROCESSED ONLINE BY THE CLUB SECRETARY. 

  1. PLEASE FILL IN THE ABOVE  DETAILS OF THE  INJURY IN THE SAME WAY AS BEFORE AND SUBMIT THROUGH THE OFFICE FOR THE CLUB SECRETARY. 
  2. WHEN YOU ATTEND A DOCTOR GET THEM TO FILL IN THE MEDICAL CERTIFICATION
  3. TO ALLOW DWF  TO PROCESS THE CLAIM FILL IN THE DECLARATION

DO NOT POST THE FORM

Please note : Physio except post operation physio recommended by a surgeon is not claimable under the GAA scheme.

Be aware of this before you engage in expensive treatment. If in doubt call the club secretary or read the handbook

LADIES

>> The LGFA have a separate Injury claim process. The main difference is that any private treatment for injury must be PRE AUTHORIZED by the fund admin see this link

Again check first with the club secretary before you engage in expensive private treatment and always submit within 60 days of injury

>> For Camogie insurance the club operates its own separate camogie insurance policy through the brokers, Willis. Please bring all camogie injuries to the attention of the Camogie Delegate   Alison Quinn or the club secretary immediately in order to process a claim


>>For Gaelic for Mothers/fathers the club has  a policy with Gogans insurance. Bring any injury to the attention of the club secretary or to Maria Gallagher , as soon as possible
